Verification Material Requirements
(1) Bring the original passport,the original physical examination record of foreigners handled by overseas public hospitals and 2 passport-size color photos of 2 inches with white background;
The first page of the original physical examination record shall have his own photo and be stamped with the cross seal. The name must be consistent with that on the passport.The last page must have the international date (AD),the seal of the physical examination institution or the signature and seal of the examination doctor;
(2) Separate original records of blood examination,chest X-ray examination and ECG examination must be provided. The recording time must be in the international common date (AD).Each original record must be signed and sealed by the examining doctor or the seal of the physical examination institution:
Blood test original record: must include syphilis,AIDS (type I + type II),hepatitis B surface antigen, hepatitis C antibody,alanine aminotransferase inspection details;
Original record of chest film examination:choose one of the following three records. a. X-ray film (size must be greater than 7 ×11cm, which must be printed with accurate personal information of the subject, information of the examiner or inspection organization and international inspection date);b.A report signed and sealed by the examining doctor;c. CD (containing accurate personal information of the subject,information of the examiner or inspection organization and international inspection date);
Original record of ECG examination: choose one of the following two records. a. A report signed and sealed by the examining doctor;b. ECG drawings signed and sealed by the examining doctor;
All records must be in Chinese or English and submitted at one time. It needs to be verified that the validity period is within six months from the signing date of the original physical examination record of foreigners and the signing date of the original examination details.
Verification Process
After submitting all the materials, they will notify you whether you are qualified or not within 5 working days. Those who are unqualified need to make up items or have a new physical examination (as required by the doctor),and those who are qualified will receive the verification report directly.
